New Patient Forms
We are pleased to have you as a new patient. Prior to your first appointment, please print the forms (Adobe Acrobat Reader is required) below and complete them and take these to your appointment. This will allow us to expedite your visit. We take pride in our short wait times - and this is how you can help shorten your visit time with us.
Please expect that your eyes will be dilated during the exam. Dilation allows you to receive the most thorough level of eye care possible. We recommend that you do not drive after being dilated.

There is a $30 no show fee if you need to reschedule your appointment but fail to do so at least 24 hours before your appointment time. By limiting no show appointments, we are able to provide all of our patients with outstanding care in a prompt manner.
